Your First Visit

On your first visit we ask that you come about 15 minutes early to complete the registration process.

When you arrive at MacGregor, please sign in at the center station of our 5 reception desks. A front desk staff member will call you as soon as possible to start the registration process. The front desk person will ask you for your insurance card and for your driver’s license, which will be scanned. We will have attempted to verify your insurance prior to your arrival, but sometimes we can only verify it on the day of your visit.

Note

Please note that your insurance company may require you to pay a copay or a deductible at the time of the visit. We do need to follow the insurance company rules, so please be prepared to take care of this at the time of your visit.

On the first visit, the MacGregor staff will ask you to complete an information sheet about yourself, such as your address, phone number, spouse name, and more. In addition, you will receive a notice of HIPAA privacy information, which we will ask you to review and then confirm in writing that you have been given the opportunity to review the information. We will also give you a form that outlines our financial policies, and we will ask you to sign this as well (you may have a copy of the HIPAA notification and the financial policy form.)

In order to review your medical history more thoroughly, we will ask you to complete a history form for yourself or for your child. This will be reviewed by the physician who sees you.

We may also ask you to give us authorization to obtain prior medical records.

In addition, you may wish to complete a Directive to Physicians (Living Will) and/or a Durable Power of Attorney for Health Care. If you have these forms already, we may ask you for a copy so we may include them in your record.

If you have a child (we will see 16 and 17 year olds) but wish to have a non-parent bring the child in, you may wish to complete an authorization on the first visit, or even prior to the first visit.

You may download these forms and complete them in advance. The HIPAA privacy information is also available to review prior to your arrival.  If you have forms that require completion by a physician, there is usually an additional fee for each form (examples are FMLA forms and disability forms).

We do request that you cancel an appointment in advance (at least 3 hours) if you are unable to keep an appointment, so that we may offer the appointment to another patient.  We do charge a fee of $50 for unkept appointments.
